This is for a copy of the Part 48 MSHA 5000-23 form. The sequence of events for this:

  1. You request a copy and pay the fee
  2. We locate you in our records
  3. We will send you a PDF copy via email
  4. If you need a "hard copy" mailed, request one when you continue the registration.

If we can't find a copy but we have enough information to make another one, that is what we will do.

If we can't find a copy and we don't have enough information to make another one, we will refund you the fee.

IMPORTANT - - If you find your copy but we have already completed our end of the work, the fee is non-refundable.

 

If we are able to provide a copy of your 5000-23, we will email you that copy. Select MAIL when you register if you need/want a Hard Copy mailed to you.
